Among general-purpose surgical robots with a published price band, the Dexter from Swiss maker Distalmotion sits at the bottom of the range at roughly $500,000 to $1,000,000 (analyst estimate, R2 Surgical 2025 pricing guide). That floor is roughly a third of a new da Vinci Xi and about half of a Stryker Mako. The catch is that "cheapest" is doing work: Dexter's low end assumes a hybrid open/robotic operating room where the hospital already owns the laparoscopic tower, and the recurring per-case instrument cost is quoted separately.

If you widen the definition to include narrow-purpose systems, cheaper handheld and semi-active robots exist (spine screws, knee resurfacing, dental implants), but none of them can do a cholecystectomy or a hysterectomy. Once you require a general soft-tissue platform that competes with da Vinci in a real OR, Dexter is the current price leader on the record.


Why Dexter wins on capital price

Distalmotion's pitch since its 2023 FDA de novo authorization has been explicit: sell a robot a mid-sized community hospital can afford. The system uses sterile, single-use instruments docked onto a compact bedside cart rather than the four-arm boom-mounted architecture of da Vinci. The surgeon stands at the patient side (open-console ergonomics) instead of sitting at a remote immersive console, which removes one of the two most expensive subsystems in the da Vinci architecture. The R2 Surgical 2025 pricing guide places new Dexter capital at $500,000 to $1,000,000, and refurbished units even lower.

That number is an analyst estimate, not a verified purchase invoice. Distalmotion, still private, has not published a list price, and hospital procurement contracts remain confidential. Treat the low end as the aspirational floor the company is targeting, not a guaranteed sticker.

  5. Medtronic Hugo RAS: price undisclosed on the public record. Medtronic has consistently framed Hugo as a value alternative in earnings calls and analyst days, but has not published unit pricing and no invoice-grade figure has surfaced.
  6. MicroPort MedBot Toumai: price undisclosed. China's leading domestic four-arm platform; NMPA approved and installed in Chinese tier-3 hospitals and now in select overseas pilots. Chinese trade press has reported unit prices roughly a third of da Vinci in yuan terms, but no auditable USD figure appears in the public record.
  7. LEM Surgical Dynamis: price undisclosed. Swiss dual-arm orthopedic platform.
  8. MMI Symani: price undisclosed. Microsurgery-specific system for supermicro anastomosis and lymphatic reconstruction, not a general laparoscopic platform.
  9. THINK Surgical TMINI: price undisclosed. Handheld knee-arthroplasty robot; widely reported in trade press as substantially cheaper than Mako on a per-installation basis, but no auditable capital figure is on record.

What beats Dexter on price but doesn't count as "the cheapest surgical robot"

Handheld orthopedic robots. THINK Surgical's TMINI is a handheld, wireless miniature bone-cutting robot designed specifically to break the capital-cost barrier of Mako and ROSA Knee. Trade press has quoted figures as low as the low six figures for the base unit, but the company has not disclosed a firm price. Even if the number holds, TMINI does one procedure family: total knee arthroplasty. It cannot substitute for a soft-tissue system in a general OR. Same story for Mazor X (spine screw placement) and the ROBODOC Surgical System. These are surgical robots, but they are single-lane. Comparing them to Dexter on price is comparing a nail gun to a machine shop.

Semi-active image-guided systems. Remebot (neurosurgery) and TiRobot (spine and trauma) from China are cheaper on paper than any Western general-purpose platform, but again, single-workflow, not general soft-tissue.

Dental implant robots. The Vantis Dental Robot and Yomi (not on the registry) sit well below $500K, and are FDA cleared, but "dental implant placement" is not what buyers mean when they price shop for a "surgical robot".

Refurbished and off-lease da Vinci. A used da Vinci Si (the previous generation) trades in the $250K to $500K range on the secondary market via brokers like R2 Surgical. This is real, but it is not a new-system price and Intuitive does not sell or service it, which is why comparison guides exclude it.

Vendor "target" prices. Vicarious Surgical has publicly targeted a system priced well under da Vinci, but the company has not shipped a commercial system as of the last reviewed record, and its own FDA path has slipped repeatedly. A price target on a robot you cannot buy does not qualify.


Gotchas and caveats

Capital is a fraction of lifetime cost. For every major platform, the recurring instrument and service costs dwarf the initial purchase over five years. Intuitive's own 10-K breaks revenue into systems, instruments and accessories, and services, and the recurring lines are consistently 80-plus percent of the total. Dexter and Versius are designed around single-use or short-life instruments where the per-case bill is the differentiator, not the sticker. A hospital chasing a cheap Dexter can spend more per case than a hospital running an amortized da Vinci Xi at 700 procedures per year. The cheapest robot to buy is not the cheapest robot to run.

"Analyst estimate" is not "verified sale price". Only the Mako and da Vinci figures above trace to auditable filings and industry procurement databases. Dexter and Versius are R2 Surgical's brokerage estimates: informed, but not a signed invoice. Medtronic, MicroPort, LEM, MMI, and THINK have chosen not to publish. When a hospital procurement office asks "what do these cost?", the honest answer for those five is: request a quote, expect confidentiality, and expect wide variance based on service term, instrument commitments, and trade-in.

RaaS and lease structures muddy the comparison. Both CMR and Distalmotion have talked publicly about robotics-as-a-service and per-procedure pricing to remove the capital hurdle entirely. Under those structures the sticker becomes moot: what matters is the per-case fee and the volume commitment. Comparing a $500K purchase to a $0 down, $2,500 per case lease requires a spreadsheet, not a headline.

Regulatory scope changes the answer by geography. Versius is deployed across the UK, EU, Australia, and India but is not FDA cleared for the US market as of the last reviewed record. Toumai is NMPA approved in China with only pilot activity outside. Dexter is FDA cleared. So the cheapest robot a US hospital can legally deploy today is Dexter; the cheapest a UK NHS trust can buy is a genuine Dexter versus Versius bake-off; the cheapest a Chinese hospital can buy is Toumai and nothing on this list comes close in yuan terms.

"Surgical" is a category, not a specification. The comparison above assumes soft-tissue laparoscopic capability. If you narrow to orthopedics, Mako is the incumbent and TMINI is the disruptor. If you narrow to spine, it is Mazor X versus ROSA. If you narrow to microsurgery, Symani has no direct price competitor. The single-word question "what is the cheapest surgical robot" only has a clean answer once you specify the OR use case.


Bottom line

For a general soft-tissue laparoscopic platform with a disclosed price and FDA clearance, Dexter from Distalmotion is the cheapest system on the record at $500K to $1M, with CMR Surgical's Versius a close second at $750K to $1M for markets that accept CE-mark deployments. Everything cheaper is either single-procedure (Mako, TMINI, Mazor X), regionally restricted (Toumai), or a price target on hardware you cannot actually buy today. And every one of those numbers is capital cost only: the recurring instrument bill is where the real dollars live.
